Waiver & Release of Liability
STAND-UP PADDLEBOARD (SUP) YOGA WAIVER & RELEASE OF LIABILITY

PARTICIPANT AGREEMENT, RELEASE AND ASSUMPTION OF RISK. PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT.
In consideration of being allowed to participate in any way in SUP Yoga classes conducted by Maura Fields and Joseph Chiarello (hereafter referred to as “Instructors”), I acknowledge and agree to the following:
Assumption of Risk:
I understand that stand-up paddleboard (SUP) yoga involves physical activity in and on water, which carries inherent risks including but not limited to: falls, equipment failure, contact with other participants, water conditions, weather, and potential drowning. I certify that I am physically fit and have no medical condition that would prevent my full participation in this activity.
Release of Liability:
I voluntarily release, forever discharge, and agree to indemnify and hold harmless the Instructors, property owners, affiliates, agents, and representatives from any and all claims, demands, or causes of action which are in any way connected with my participation in SUP yoga activities, including any claims which allege negligent acts or omissions of the Instructors.
Emergency Medical Treatment:
In the event of an emergency, I consent to receive medical treatment deemed necessary. I agree to be responsible for any medical costs incurred.
Personal Property:
I acknowledge that I am responsible for my personal belongings during the class and that the Instructors are not liable for any loss, damage or theft
Weather Policy:
I understand that outdoor classes are subject to weather and safety conditions. Classes may be delayed, rescheduled, or canceled due to unsafe weather or water conditions at the sole discretion of the instructors.
Age Requirement:
Participants under 18 years of age must have a parent or legal guardian sign this waiver on their behalf
